Information and forms for sponsors and students of approved placements for 2012 SWASP Community Service Component. Completed forms can be faxed to CSC SWASP (709-726-9125) or mailed to CSC SWASP (Suite 201 Virginia Plaza, St. John’s, NL A1A 3E9).

Forms

Student Cancellation Notification Form
Sponsors submit this form if a student starts work but leaves the placement early.

Stipend Disbursement Form
This form must be signed by the student each time a stipend payment is made.

Declaration Form
The Declaration Form must be signed by both student and sponsor and sent to CSC during the 5th week of the placement. The tuition voucher will be issued and sent to the sponsor only after CSC receives this form.

Sponsor Comments Form / Student Comments Form
The Sponsor and Student Comments Forms provide important feedback on SWASP.  These are to be completed at the end of the placement.
